I decided to use the same formula used in the North Vision Ranking of Nations and use it for Eurovision! 
Here is the result:
Method:
1. Take a country's average number of points gained throughout 2012 and multiply the value by 0.25
2. Repeat for a country's score in 2013 but multiply by 0.5 instead
3. Repeat for a country's score in 2014 but multiply by 0.75 instead
4. Repeat for a country's score in 2015 but keep the value the same
5. Add up all the values found in the first 4 steps
6. Round the value to the nearest integer (whole number)
These results gained after 2015 are then compared to the 2014 ranking (which uses the 2011 - 14 editions), hence the "+/- Rank / Points"
